Creating website submissions for Squarespace

There are three essential components to setting up a submission process on a Squarespace website: a graphic (designer), some code (developer), and a form (Jotform). We are using a service called Cloudinary to receive and store submissions and to generate URL’s for our forms. The developer’s code allows us to take a video file uploaded to Cloudinary, generate the URL, and put them into a hidden field in the JotForm. Even if you aren’t able to code, you still should be able to duplicate the process from other forms.

  • When in JotForm, fields can be shown or hidden. When creating a new form, duplicate in Jotform to copy over hidden fields.

  • In Squarespace, add a code block and add form as embed code.

  • From here, you’ll need to go back to Jotform. Go to the relevant form and click edit form.

  • Click publish.

  • Click embed. Code should look like the image below.

  • Copy this code from Jotform into the code block from Squarespace.

  • That’s it. You should be good to go.
